Service Description
Avena Diagnostics is a mobile specimen collection service. We provide blood draw services at your home, workplace, or other approved location. We do not diagnose, treat, or interpret medical conditions or lab results.
Lab Order Requirement
A valid lab order from a licensed healthcare provider is required prior to specimen collection. Clients are responsible for obtaining and providing accurate lab orders.
Payment & Pricing
All services are self-pay. Pricing is listed at the time of booking and may vary based on service type and location.
A deposit is required to secure your appointment and will be applied toward your total service cost. The remaining balance is due at the time of service unless paid in full online.
Cancellation & No-Show Policy
Appointments must be canceled or rescheduled at least 24 hours in advance.
Deposits are non-refundable for:
- Cancellations made within 24 hours
- Missed appointments (no-shows)
Clients who are more than 15 minutes late may be considered a no-show and may need to reschedule.
Client Responsibilities
Clients agree to:
- Provide accurate and complete information
- Ensure a safe and clean environment for service
- Follow any preparation instructions (fasting, hydration, etc.)
Limitation of Liability
Avena Diagnostics is not responsible for:
- Lab processing errors or delays
- Results interpretation
- Provider-related issues
We are only responsible for proper specimen collection and handling.
Right to Refuse Service
We reserve the right to refuse or discontinue service if conditions are unsafe, inappropriate, or outside the scope of our services.
